Improving End of Life Conversations with Patients

Dear Coalition Members,

The Ventura County Coalition for Compassionate Care participated in a panel about Goals of Care at the Annual Conejo Valley Caregiver Recognition Day.  The panel was highly valued at the event. Therefore, we want to offer it again. Both Dr. Reyes and Laura Zempel actively interact with patients and caregivers about serious illness, goals of care and the need for healthcare providers and teams to introduce and promote conversations about goals of care.
